Can someone please tell me how I make this type of form, Safe Auto Insurance Quote - Safe Automobile Rates or better yet, what is it called so I can just go find a tutorial elsewhere and leave you all alone

I want to use an image I made, but insert that place "enter your zip code" so that it collects there info and then the 'submit' button will forward onto an affiliate program taking the info typed into the graphic with them??? The one I'm showing you is fake I know, you can't really input your zip, but I want one you actually input the info before you go to the affiliate site.

For example, if I use this type of picture, I then want to learn to add the zip code or whatever fields under the ,1,2,3 etc. and that info must then be carried with parameters to the next site after you hit submit.

Not that I know what I'm talking about, but have a required field, and have that value passed along with the POST to the next page or whatever when it is submited or "get my quote" button is clicked. Sound about right?

Thats why your the coder and I'm the designer :headbang: I don't know how to get thing to work, but I can make them look good LOL :rofl:
